This is where the Deceased’s relationships are added.
In the screen shot below I have highlighted the ‘Relationship Status at Death’ area.
- This is the deceased’s status at death.
- You are only able to choose one of these options.
- The Deceased will fall into one of these categories.
- This is the state of the deceased at the time of death.
- This state does not change when entering previous relationships. It remains the same.
You must select what type of relationship it was. This pertains to the relationship tab you are on and does not affect the ‘Relationship Status at Time of Death’ (highlighted above).You must choose the sex of the spouse or partner even if the relationship is a marriage.By entering the Spouse’s Date of Birth in the Optional Box and tabbing out of it – FreeDAM will then calculate the spouse’s Age for you. (This date is not sent to BDM)By entering the Date of the Relationship Formalised in the Optional Box and tabbing out of it – FreeDAM will then calculate the Age of the deceased at the time of the event for you. (This date is not sent to BDM)Only the name of the Town or City is required – the region is not. For example Crawley gets entered in the Town Field NOT ‘Crawley,West Sussex’ or ‘Crawley, England’Of course ‘England’ would be entered in the Country field (leave the dash there instead of ‘New Zealand’)Enter any other relationships in order of the most recent through to the earliest. Remember – changing the relationship Type in a previous relationship should not change the status at the time of death.
The last field for data on this screen is the question concerning the deceased holding and honour or award..
The fields and buttons beneath the lower horizontal line are common to all tabs and concern the overall death certificate.
