This tab is used to capture additional data about the deceased.
When you click ‘Edit’ – FreeDAM will place your cursor in the first field for you ready to receive data.
Note that in the image above the cursor is in the third ‘Cause or causes of death’ field.This is because I ‘tabbed through the first two fields ‘Deceased’s first Name/s at Birth’ and Deceased’s Surname at Birth. FreeDAM puts a ‘-‘ in fields that we have entered and left empty indicating that we have paid attention to those fields and no data is to be entered in them. (The dash will not be sent to BDM or show on the certificate)Usually the first names at birth do not change – we only put information in this field if they have changed since birth – otherwise tab through it.
The Deceased’s Surname at birth may very well be different if the deceased is female and has married. In this case her maiden name would go in this field. Otherwise tab through it.Enter the deceased’s causes of death in the appropriate fields.
Comma’s, Hyphen’s and full stops are not allowed.
If the causes of death are too long for the field

If the causes of death are too long for the field
- Enter as much data as you can and put a ‘?’ (question mark) at the end.
- Goto the ‘BDM Order Info and Comments’ Tab
- In the comments field put in the field name (‘Cause of death part 1a’) followed by ‘continuation’ and then the remaining data for that field.

The fields and buttons beneath the lower horizontal line are common to all tabs and concern the overall death certificate.
